Lurtz and Brego were utter fabrications of the films and (to my best knowledge) Frodo's index finger was never specifically featured outside the movie.
Gothmog (being a slight red herring) WAS mentioned in the book but it was never specified that he was an Orc. In fact, I always assumed he was a human.
